Thank you for nominating a great American woman for consideration of induction into the National Women’s Hall of Fame. Before completing this form, please be sure to review the eligibility criteria.

We are currently accepting nominations for consideration in the 2017 Induction cycle. Nominations received before January 31, 2016 will be considered for the 2017 cycle.

Paper Form: Download the Word document below and complete the form. The form must be typed; handwritten nominations will not be accepted. A list of references and citations will not be considered complete biographical information.
